Accutane 3 months still breaking out

Why am i wellbutrin and concerta combination breaking out on accutane after 4 months? I have been on accutane for 4 months now, specifically 4 "accutane 3 months still breaking out" and 2 weeks, and am still experienced cystic acne. I am currently taking 30 mg once a day. I was told that for the first months i will breakout with acne, but after 3 months it would go away. However this is not the case.
