As a physician with an interest in reducing opioid-related problems, I frequently hear stories from colleagues and friends about their loved ones who either struggle with opioid addiction or have even died from opioid-related overdose. My follow-up question to them is usually: He went in for a minor back surgery and was discharged with 60 tablets of oxycodone the opioid medicine in Percocet and Oxycontin. For the first couple of days, he had significant pain and used the pain pills. After the pain began to subside and acetaminophen and ibuprofen were sufficient, he stopped using the oxycodone. But after discontinuing the medication, he began feeling terrible, experiencing body aches, restlessness, and insomnia. He took another oxycodone and felt better. Fortunately, he had the insight to recognize what was going on: Had he continued treating his withdrawal with oxycodone, he may have become hooked.

Spinal procedures are generally associated with intense pain in the postoperative period, especially for the initial few days. Adequate pain management in this period has been seen to correlate well with improved functional outcome, early ambulation, early discharge, and preventing the development of chronic pain. A diverse array of pharmacological options exists for the effective amelioration of post spinal surgery pain. Each of these drugs possesses inherent advantages and disadvantages which restricts their universal applicability. Therefore, combination therapy or multimodal analgesia for proper control of pain appears as the best approach in this regard. The current manuscript discussed the pathophysiology of postsurgical pain including its nature, the various tools for assessment, and the various pharmacological agents both conventional and upcoming available at our disposal to respond to post spinal surgery pain. Baring few exceptions, spinal surgeries are mostly elective in nature. Conventional spinal surgeries nonminimally invasive often involve extensive dissection of subcutaneous tissues, bones, and ligaments and thus result in a considerable degree of postoperative pain.

On the morning of October 23, I awoke to a stiff neck, attributing this to sleeping the wrong way. I went out for my normal run that morning, but the stiff neck didn't improve.
